Will you file a claim with my insurance company, or is that my responsibility?
Payment in full is requested at the time of service. As a courtesy to our patient, we will give you a completed itemized statement to file with your insurance for your reimbursement.
Where are you located?
Our office is located at 345 W. Portal Avenue, 2nd floor, in the Sunset district of San Francisco, California. (see map)
Where should I park?
There is ample free parking located at the back of our building. Our clinic is also accessible through public transportation.
What age limit is there to have an anti-aging evaluation?
Most people who start our anti-aging program are 40 years old and above. If however, there is a personal or family history significant for chronic degenerative diseases, an evaluation may be done sooner. There is no upper age limit to start a rejuvenation program. If you are interested in improving your health in general, we can design a program that suits your needs.
Payment Information
Alternative/Complimentary/Holistic or Anti-Aging Medicine is preventive medicine practiced at it's highest level and therefore, insurance plans, particularly HMO's, do not pay for this service. Al patients are requested to make full payment at the time of our service. We take cash, check, Visa or Master Card.
If you have PPO or private insurance that allows you to see a doctor outside the network, we will give you an itemized statement that you can submit for possible reimbursement.
If you are a Medicare patient (provided that you don't have an HMO as a secondary insurance), after paying us the full amount, we will submit the office visit charge to Medicare and they will send the reimbursement directly to you.
